Tension-type headache (TTH) is the most common type of primary headache, characterized by a dull, aching pain and a sensation of tightness or pressure around the head. These headaches can vary in frequency and intensity, ranging from episodic to chronic.

Characteristics

  • Pain Quality: Dull, aching, and non-pulsating.
  • Pain Intensity: Mild to moderate.
  • Pain Location: Typically bilateral, affecting both sides of the head, and often described as a tight band around the forehead or back of the head and neck.
  • Duration: Can last from 30 minutes to several hours; chronic TTH can persist for days.

Types

  • Episodic Tension-Type Headache (ETTH): Occurs less than 15 days per month.
  • Chronic Tension-Type Headache (CTTH): Occurs 15 or more days per month for at least three months.

Causes and Triggers

The exact cause of TTH is not fully understood, but potential factors include:

  • Muscle Tension: Strain or tension in the muscles of the neck and scalp.
  • Stress: Emotional or mental stress is a common trigger.
  • Poor Posture: Contributing to muscle strain.
  • Fatigue: Physical or mental tiredness.
  • Eye Strain: Prolonged use of computers or other devices.

Diagnosis

Diagnosis is typically based on clinical history and symptom description. There are no specific diagnostic tests for TTH, but imaging studies like MRI or CT scans may be used to rule out other conditions.

Managing tension-type headaches often involves a combination of medication, lifestyle modifications, and addressing any underlying stressors or physical issues.

What are the signs and symptoms of Tension Type Headache?

Tension Type Headache is usually felt by a constant pressing, tightening and squeezing discomfort on both sides of the head, as well as aching or tight neck and shoulder muscles. Sufferers normally describe their head pain as if they were wearing a tight helmet, which can become tighter and tighter around their head. The pain intensity can range from mild to moderate and can last anywhere between 30 minutes and up to 7 days. Nausea or vomiting is not a feature of tension type headache however, sensitivity to light or sound can be.

  • Chronic Tension-Type Headache (CTTH)

Episodic Tension-Type Headache is defined by the number of headache days experienced each month.

Typically, Episodic Tension Headaches occur if an individual has fewer than 15 headache days per month over a period of at least three months. These headaches can last anywhere from 30 minutes to 7 days.

Recent international research indicates that the average prevalence is 36% for men and 42% for women. If Episodic Tension-Type Headache worsens over time, it may develop into Chronic Tension Headache.

Chronic Tension-Type Headache is identified by the number of headache days experienced each month.

In contrast to episodic tension headache, chronic tension headache occurs when a person has more than 15 headache days per month for at least three months. Some individuals may experience a continuous headache lasting all day for 10-30 years or more.

Chronic Tension-Type Headache is typically resistant to pain medication, and sufferers may also experience chronic migraine alongside their chronic tension headaches.
